The Unit has a recognized rotational training program for resident anesthesiologists in training and takes active part in post-graduate seminars both in Israel and overseas. A particular interest of the Unit is the intra-operative diagnosis and management by means of trans-esophageal echocardiography - a service which is expanding as rapidly as technology allows. The Unit brings together the collective post-graduate experiences gained from Israel, the U.S.A., Australia and England. The head of the Unit is Dr. Jeffrey Fisher. Dr. J. Fisher. Dr. Jeffrey Fisher M.D., Dr. J. Fisher M.D., The Gastroenterology Institute headed by Dr. W. Jacobsohn, comprises a referral clinic of 6000 patients a year; Shaare Zedek, an endoscopic unit (headed by Dr. D. Braverman) performs 4000 examinations a year; Shaare Zedek, and a G.I. Laboratory (headed by Dr. R. Goldstein) is engaged in clinical and research work which provides the necessary tools for the management of G.I. patients. (C-13 breath tests and absorptive studies.) Five senior gastroenterologists (Drs. Jacobsohn, Braverman, Paz, Morali and Zeides) work in the department. A pediatric gastrointestinal unit is headed by Dr. Y. Faber. The endoscopic work is performed by the pediatricians at the endoscopy unit. Endoscopic procedures are performed on a daily basis. Laser photoablation, stent insertion, Shaare Zedek, dilatation of strictures, injection sclerotherapy etc., are routinely performed. ERCP's are done twice weekly and Motility+pH studies one day a week. A hemorrhoid clinic takes place almost every day. A surgeon joins the clinic once weekly. A percutaneous feeding gastrostomy program is provided for the geriatric institutions in the city. A liver clinic is held weekly by Dr. Morali, in collaboration with Professor Halevy - Director General of the Medical Center and a specialist in liver diseases. A specialized clinic for the early detection of colo-rectal cancer provides screening and genetic counseling to high risk individuals. Clinical research facilities are available. Articles in various fields of interest have been published in the literature. A fellowship program and a resident program are available. A foreign-student program is also available for short and long terms. Our 6-bed Intensive Care Unit (ICU) provides the outmost multidisciplinary medical care of critically ill patients. We try to help the patients and their families overcome the highly stressful time in the ICU, not only by modern medicine, but also by moral support and human attitude. Some patients spend many weeks with us and the staff becomes an integral part of their family and the ICU becomes their "home". The main bulk of work in the unit is done by the ICU nurses who are not only highly qualified professionals, but also highly sensitive to human integrity and needs. The unit is run by Dr. Moshe Hersch, certified Internist and Intensivist, educated in the Shaare Zedek Medical Center and in Canada. Dr. Hersch's top priority in his attitude is not only to provide the highest medical standards for his patients, but also to practice intensive care with compassion and utmost consideration to the patient's and family's psychological needs. Dr. Moshe Hersch was granted the "Exemplary Doctor" Award by the Israel Society of Medicine and Law 1994. The endless chain of gratitude letters sent to the ICU attest to the Unit's success. tHE ADEPRTMENT OF INTERNAL MEDICINE B The Department consists of 32 Acute Medical beds, the vast majority of the patients being direct admissions from the Emergency room. Thus all aspects of Medicine and its subspecialties are covered giving a wide experience to the interns and residents. Great emphasis is placed on physician/nurse-patient relationships and total patient care and the Department motto being TLC - "tender, loving care". The medical staff consists of: The Director - Professor Abraham S. Abraham, MD, FRCP, The Deputy Director - Ovadia Shemesh, MD, A Senior Attending - Gidi Nesher, MD, 6 residents & 1-2 interns. Undergraduate students from the Hebrew University-Hadassah Medical School are taught most of the year. Research interests are: Magnesium in Acute Myocardial Infarction, Rheumatoid Arthritis, Temporal Arteritis, Antiphospholipid syndrome. Medical Halachah. PROFESSOR ABRAHAM S. ABRAHAM, MD, FRCP Abraham S. Abraham was born at the Shaare Zedek Hospital in Jerusalem and received his medical training at the Leeds School of Medicine, England where he graduated MB,ChB in 1960. In 1968, as a result of pioneering work on the effects of oxygen in chronic bronchitis, he was awarded the MD by the Univ. of Birmingham (England). This work won him the annual prize of the British/American Heart Fellowship - a year as a Senior Research Fellow at the Johns Hopkins, Baltimore. In 1977 he was elected a Fellow of the Royal College of Physicians (London). Since 1989 he has been the Chief of the Department. In 1990 he was elected a Professor of Medicine at the Hebrew University/Hadassah Medical School (Jerusalem) & has been included in the list of outstanding teachers on a number of occasions. He is a member of the Hebrew University Senate and an examiner for the Boards in Internal Medicine. He has published over 80 articles in international medical journals (including 3 in the New England Journal of Medicine) on a variety of subjects, including Pulmonary hypertension in chronic bronchitis, Magnesium in acute myocardial infarction and Chromium in atherosclerosis, and has received the Hadassah Medical Faculty prize for outstanding research.  In the field of Medical Halachah, Professor Abraham has published The Comprehensive Guide to Medical Halachah in English. Recently, two pocket books, for the physician and patient respectively, concerning the Sabbath and Festival Laws have been published, both in English and in Hebrew. His magnum opus is the 5 volume Nishmat Avraham (in Hebrew), today the classical text used by physicians, laymen and Rabbis alike. Dr. Hurwich had been trained and certified as a specialist in the United States and had been active in early, pioneer developments in the field. On October 25, 1970, the first hemodialysis treatment was performed. The Dialysis Unit has developed since then into a modern facility, offering a range of types of treatment, with its emphasis on hemodialysis and chronic ambulatory peritoneal dialysis. The Dialysis Unit, in conjunction with other departments in the ospital such as the Intensive Care Unit, is involved with developing new techniques of therapy, and continues to stand in the front line of its field. It attracts patients from all sections of the Jerusalem community, as well as accommodating tourists from overseas whom we are happy to serve during their visit to Israel. Over the years, the Nephrology Service has served in a consultative role to all the departments of the hospital and has, at varying times, emphasized aspects in which it was especially involved, with an emphasis on hypertension. In addition to its Director, Dr. Yosef Plaksin and Dr. Yitzhak Slotki serve as Senior physicians in the department. They as well as the Director are certified specialists in Nephrology and Internal Medicine. Clinic: 6513975 (appointments), SERVICES, General chest clinic - in this clinic we see children with asthma, BPD, congenital anomalies, gastro-esophageal reflux, interstitial lung diseases, children with chronic or recurrent respiratory infections, and other bronchial and lung diseases in children. Cystic fibrosis center. This ltidisciplinary center includes specialists from various medical and paramedical fields including pulmonology, gastroenterology, nutrition, social work, and physiotherapy. We take care of nearly 50 CF patients. Day Hospital - Center for therapy for children with chronic lung diseases, children on home oxygen therapy and children on home entilator therapy. Sleep and breathing disorders Clinic - in this clinic we evaluate and treat children with sleep disordered breathing (obstructive and/or central apnea), stridor, hoarseness ALTE survivors, and patients on apnea onitors. This is a joint clinic together with an Ear Nose & Throat specialist and a pediatric neurologist. Pulmonary nction Laboratory performs spirometric and lung volume measurements, methacholine and exercise challenge tests, and assessment of respiratory muscle strength. Infant Pulmonary Function Laboratory measures passive pulmonary function and lung volumes in infants. Bronchoscopy Suite - Over 100 fiberoptic flexible bronchoscopies are performed each year in our center. The indications are suspected congenital or acquired anomalies of the irways, foreign bodies, and diagnosing infectious causes of respiratory illness in immunocompromised children. espiratory Electrophysiology Laboratory for measurement of nasal potential difference - This is a new diagnostic test for cystic fibrosis and a research aid for the effect of various therapies in cystic fibrosis & RESEARCH ACTIVITIES Cystic fibrosis The relationship between genotype and phenotype in cystic fibrosis and factors which determine the prognosis of patients with cystic fibrosis. This work is done in collaboration with Dr. Batsheva Kerem and her group at the Genetic Department, Life sciences Institute, the Hebrew University, Jerusalem, and in collaboration with all the CF clinics in Israel. In this work several uncommon CFTR mutations have been found to be more common among Jews and Arabs in Israel. In each mutation the phenotype has been defined. Subsequently, the incidence of CF in Israel has been determined, and the frequency of the different mutations among the different ethnic origins defined. In particular we are interested in splice sites mutations. Shaare Zedek, We Shaare Zedek, recently defined the clinical manifestations of patients with atypical CF who carry a splice variant in intron 8. We found that these mutations were associated with variable levels of normally spliced mRNA and this correlated with the severity of their lung disease. Related on going projects: Shaare Zedek, Shaare Zedek The prevalence of and risk factors for liver disease among CF patients in Israel. We found that the incidence is relatively high (near 30%) Shaare Zedek and the major risk factors are the type of mutations and pancreatic function. Microbiological aspects of CF and the association with genotype; comparison of severity of disease among patients and their siblings carrying the same genotype. Shaare Zedek The aim of these studies is to identify factors which contribute to phenotypic variability among patients carrying the same genotype. Potential difference measurements in the nasal mucosa. These measurements will be used to diagnose atypical CF patients carrying rare mutations and having normal or borderline sweat chloride levels. Shaare Zedek, study the physiological response to different medications. Nasal potential difference measurements among patients carrying the 5T allele. The role of nasal potential difference measurements in patients with questionable CF. Broncho-alveolar lavage in infants with CF. Findings during routine abdominal ultrasonographic studies in CF patients. Significance of provocation studies in CF.  Pediatric Asthma Use of spacer devices in acute and chronic asthma. We found that inhaler attached to a spacer was as effective as a nebulizer in children with acute asthma, Shaare Zedek and that the use of steroid inhalers attached to spacers significantly reduced adrenal suppression in children with chronic asthma compared to those who inhaled it directly from the inhaler. Shaare Zedek Adrenal suppression in children with chronic asthma using dry powder corticosteroids inhalations compared to inhalers of corticosteroids attached to spacers, and comparing adrenal suppression of various inhaled steroids preparations. Shaare Zedek Epidemiology of asthma mortality in Israel. Other research projects Pulmonary involvement in Gaucher Disease, and the response of lung function to the specific enzyme therapy (ceredase). Epidemiology and smoking behavior among schoolchildren in Israel. Currently we study the differences in the incidence of smoking and smoking behavior among schoolchildren in different ethnic populations in Israel. The efficacy of rticosteroids in the treatment of bronchiolitis - a double blind placebo controlled study. PEDIATRIC SURGERY UNIT The Unit of Pediatric Surgery has 19 patient beds and qualified nursing staff. The Unit serves all the surgical sciplines in the hospital: General pediatric surgery; Urology; ENT; Orthopedic surgery; Plastic surgery; and Eye urgery. the General Pediatric Surgery staff operates on all pediatric surgical conditions - starting with complicated  anomalies through other urgent and elective procedures. These operations are taken care of by residents and two pediatric surgical consultants. The Unit works in close relationship with the Pediatric Department; Neonatal Intensive Care Unit; Pediatric Intensive Care Unit and a team of anesthetists specializing in Pediatric Surgery. The Department of Anesthesia at Shaare Zedek Medical Center is headed by Dr. David Olschwang. Shaare Zedek Shaare Zedek Shaare Zedek Shaare Zedek It has 17 members of whom only 2 are residents. Most of the work is therefore carried out by board certified (consultant) anesthesiologists. We operate 12 operating rooms, all fully equipped with modern anesthetic machines and monitoring systems. Shaare Zedek, Shaare Zedek, Shaare Zedek, All types of surgery are carried out except for neurosurgery. There is a very busy Day-Care Center. We have a very active labor and delivery room with 5 - 6,000 deliveries a year, of which 40 to 50 percent receive obstetric analgesia administered by our department. We run an extremely busy preoperative clinic daily and also a pain clinic which is headed by Dr. Elik Vatashsky. We Shaare Zedek, have subspecialty sections: Cardiac - headed by Dr. Jeffrey Fisher ( a very active section, with full monitoring facilities, including TEE); Obstetrics - Dr. Robert Berko; Pediatrics - Dr. Benjamin Hoffmann; and Post-Anesthetic Care Unit - Dr. Yechezkel Zigelman.  SURGERY 'A' , The Department of General Surgery has 25 patient beds and a staff of 10 senior surgeons and residents. It takes care of the entire range of general surgical conditions, with special emphasis on diseases of the ro-intestinal tract, the breast and the thyroid, using conventional and laparoscopic surgical techniques. The Department tries to establish close relations with the community by conducting a large number of consultations in primary clinics, in attendance with the primary physicians, and by running the central breast clinic in jerusalem.
